Comment Card #1

I provided management consulting to the leadership team of a $11 million company. I sensed ambiguity about the results, particularly from the CEO, who was a big part of the problem. Lacking both vision and leadership competencies, he wreaked havoc in his own organization.

I decided to survey the team and show them the results, thus showing that something positive was happening. The three letters in this series tell the story.

First an e-mail asking the team to participate in the survey. Second, a sample comment card [customer service form]. Third, a letter explaining the results.

Rodrigo,
If you'll remember, we talked about the importance of "Comment Cards" in our Friday offsite, and they are especially important to me in my consulting business. Please take five minutes to mark the attached "Comment Card," and return it to me today, or as soon as practical. This will help me improve my service and effectiveness. I will tabulate the results and share them with the group.
  1. You may fill out the Word form and return it to me by e-mail, or print and FAX it to 303-555-1212.
  2. Use numbers (8, 9, 10) in the boxes, not Xs. There's a sample filled-out form at the bottom of the attachment.
  3. Signing your name is optional—not required—but your name helps me understand you better.
  4. Complete the form by close of business today, or as soon as practical.
