Thank You For Completing Comment Card

Ben was on a management team I was consulting for. In an offsite leadership meeting we had discussed the importance of getting comment cards from customers; so I mentioned that exchange when requesting his feedback about me. I sent questionnaires to all members of the management team, along with this e-mail. First is my original e-mail to Ben, then a thank-you to him upon completion. Simone is Ben's wife.

Ben,
If you'll remember, we talked about the importance of "Comment Cards" in our Friday offsite, and they are especially important to me in my consulting business. Please take 5 minutes to mark the attached "Comment Card," and return it to me today, or as soon as practical. This will help me improve my service and effectiveness. I will tabulate the results and share them with the group.
  1. You may fill out the Word form and return it to me by e-mail, or print and FAX it to 303-790-0606.
  2. Use numbers (8, 9, 10) in the boxes, not Xs. There's a sample filled-out form at the bottom of the attachment.
  3. Signing your name is optional—not required—but your name helps me understand you better.
  4. Complete the form by close of business today, or as soon as practical.

After Ben returned his form to me, I sent this note:
