Ben was on a management team I was consulting for. In an offsite leadership meeting we had discussed the importance of getting comment cards from customers; so I mentioned that exchange when requesting his feedback about me. I sent questionnaires to all members of the management team, along with this e-mail. First is my original e-mail to Ben, then a thank-you to him upon completion. Simone is Ben's wife.

- Use numbers (8, 9, 10) in the boxes, not Xs. There's a sample filled-out form at the bottom of the attachment.
- Signing your name is optional—not required—but your name helps me understand you better.
- Complete the form by close of business today, or as soon as practical.
